Registration
Registration Deadline: February
17, 2003
The
participant registration fee of $200 includes the welcoming
reception on Friday; breakfast and refreshment breaks on Saturday
and Sunday; lunch on Saturday, and the reception and dinner
on Saturday evening. Participants include faculty, residents,
and medical students entering anesthesiology. The spouse/guest
registration fee of $100 includes all of the above functions
except refreshment breaks. Registrations received after February
17, 2003, will be at the rate of $225 and $125, respectively.

A
separate registration should be submitted for each participant.
For security purposes, credit card information should not
be submitted via e-mail or Internet.
Registrations
must be accompanied by full payment. Credit card payments
may be made by MasterCard and Visa. Checks and money orders
are payable to the University of Illinois. Fax orders are
accepted if paid by credit card. Phone orders cannot be accepted.

Refunds
Registration
cancellation and refund requests must be confirmed in writing.
Requests received by the registration deadline, February 17,
2003, are subject to a $50 administrative fee. No refunds
will be issued for requests received after that date, or for
non-attendance; however, substitutions will be accepted. In
the case of a substitution, the Office of Conferences and
Institutes should be contacted to confirm the new participant.
